When the social security number is incorrectly reported to the Clerk's office or the non-filing spouse has not been added to the original form, an Amended Statement of Social Security Number must be filed. Local Form 121 should be used for this amendment.
Filing Requirements:
- Amended Form 121 - Statement of Social Security Number(s) with ATTACHED Proof of Service reflecting all creditors received notice of the full un-redacted social security number for the debtor. **NOTE: If Form 121 is Amended to add a non-filing spouse, a proof of service is not required.
IMPORTANT: The Proof of Service must be ATTACHED to the Amended Form 121. (A proof of service is not required if only adding non-filing spouse.) When filing electronically, use the Amended Statement of Social Security Number event to ensure confidentiality of the full SSN.
To File Electronically

Log into CM/ECF.

Select [Bankruptcy > Miscellaneous]; .

Select [Amended Statement of Social Security Number] from the event list.

Select the party filer.

Browse to select the Amended Statement of Social Security Number with the attached Proof of Service (pdf file).

Verify the final docket text; if correct, click [Next] to submit your document(s).
