Supplemental Creditor Matrix (aka Mailing Matrix or List of Creditors)

When a Supplemental Creditor Matrix is filed adding additional creditors, a $34.00 filing fee is required unless the case has just converted.

Converted Cases: Additional creditors may be added at no charge if the schedules and matrix supplement are filed within 14 days of the conversion.  A $34.00 fee will be assessed for adding creditors more than 14 days after the conversion.

Note: When the Supplemental Creditor Matrix is filed with Amended Schedules D, E, and/or F, the matrix is added as an attachment to the Amended Schedules entry. Refer to instructions for Amendments to Schedules.

To File Electronically:

1. Log into CM/ECF.

2. Select [Bankruptcy >  Miscellaneous].

3. Enter the case number (e.g., xx-xxxxx).

4. Select [Supplemental Creditor Matrix (aka List of Creditors) - Filing Fee Required] from the event list.

5. Select the party filer.

6. Browse to select the Matrix (pdf file).
   
   Note:  Matrix must be in .pdf format.

7. Check box 'Add new creditors' to add new creditors to case.

8. Enter name and address of each new creditor. Address may be up to 4 lines. More than one creditor may be entered. Separate creditors with a blank line. Creditor type is Creditor. Creditor committee should be No.
   
   Tip: You can copy and paste the creditor information from your original document.

9. Answer question, "Do you certify that all additional creditors have been added to this case?"

10. To pay via credit card, leave the receipt field blank and select NEXT to continue filing.
    
    Note:  If case has just converted to another chapter, and debtor is filing schedules, type "CONV" in receipt field to avoid a charge to your creditor card.

11. Modify the text as appropriate.

12. Verify the final docket text; if correct, click [Next] to submit your document(s).
    
    Note: The Notice of Electronic Filing displays giving you the document number. Copies of this notice are immediately e-mailed to all participants who receive electronic notification in the case.
