Chapter 11 small business or subchapter V are required to file monthly financial reports.
The Small Business Monthly Operating Report must be filed using the Official Form 425C, not UST Form 11-MOR, in a small business or subchapter V bankruptcy case in which a plan has not yet been confirmed. This form can be downloaded from the U.S. Courts website.
A copy of the report must be filed with the court.
To File Electronically:

Log into CM/ECF.

Select [Bankruptcy > Miscellaneous].

Enter the case number (e.g, xx-xxxxx).

Select [Small Business Monthly Operating Report] from the event list.

Select the party filer.

Browse to select the Operating Report (.pdf file).

Enter filing period (e.g., "July 2004" or "period ending July 31, 2004").

Modify the text as appropriate.

Verify the final docket text; if correct, click [Next] to submit your document(s).
Note: The Notice of Electronic Filing displays giving you the document number. Copies of this notice are immediately e-mailed to all participants who receive electronic notification in the case.
